We try our best to verify your insurance benefits as accurately as possible. The information we receive from your insurance provider regarding coverage is only an estimate. There are usually many variables set by your employer and insurance company that determine actual coverage.
Any outstanding costs not paid by the insurance company after 30 days regardless of coverage or reason, automatically become the responsibility of the patient or guardian.
PAYMENT FOR SERVICE:
Payment for any treatment and care rendered to you is due upon arrival for services. Our office accepts, cash, Visa and MasterCard. We also accept third-party financing from CareCredit and Spring Stone. Checks are accepted at the discretion of the office. There is a $35 fee for insufficient funds.
At Sienna Smiles, we are aware that in life things happen, and we understand emergencies. However, our schedule does not allow for a patient who repeatedly cancels appointments.
